Nausea and vomiting is a common and distressing symptom or side effect in medicine, surgery and following anaesthesia. It can cause complications such as wound dehiscence, electrolyte imbalance, increased pain, dehydration and aspiration. Generally, uncomplicated PONV rarely goes beyond 24 hours post-operatively. Problematic PONV however is more multifactorial in origin and can be difficult to treat effectively. Patients at risk of this should be identified by the anaesthetist and may be given prophylactic anti-emetic treatment. Post-operative patients with nausea and vomiting may be considered as either failure of prophylaxis or for primary treatment. Drug therapyThe table below is a general quick guide on the prescribing of anti-emetics, but see local guidelines.
General notes
Other information
What is used for treatment of postoperative nausea?Commonly used older, traditional antiemetics for PONV include the anticholinergics (scopolamine), phenothiazines (promethazine), antihistamines (diphenhydramine), butyrophenones (droperidol) and benzamides (metoclopramide).
What drug prevents postoperative nausea and vomiting? For people at higher risk, we found that some antiemetic medicines work well to prevent them from being sick after general anaesthesia. The best antiemetic medicines with reliable evidence were aprepitant, ramosetron, granisetron, dexamethasone, and ondansetron, followed by fosaprepitant and droperidol.
Which class of antiemetics is used for acute vomiting?Dopamine antagonists such as prochlorperazine or chlorpromazine are effective in controlling nausea and vomiting.
Why is ondansetron used after surgery?Ondansetron was administered at the end of surgery based on the results of Tang's study. They found that ondansetron administered immediately before the end of surgery was most efficacious in preventing postoperative nausea and vomiting in out-patient laparoscopy (16).
